Frequently Asked Questions

Select another doctor (neurologist) as soon as possible.  You can find them by searching online or contacting local county offices.  We already sent a list of doctors in this market to the patients we have seen in the last couple of years.  You are welcome to contact our office for any questions or guidance.

Once you select a doctor - please fill out a form to authorize release of your records to that doctor of your choice.  Mail or Fax that signed form to our office.  Once we receive that form we will transfer your records to the doctor of your choice.

We will be open until February 28, 2022 and we will continue to see the patients that are needed to be seen until then, and take care of their needs. 

Your medical records will be turned over to a custodian company - the details of which will be posted on this website.
